FutureBright Care Navigator Inc. (“Care Navigator”) provides medical care co-ordination services which assist individuals in navigating the health care system and obtaining medical help. FutureBright Insurance Group (“FutureBright”) is a company related to Care Navigator. FutureBright distributes critical illness, accidental death and dismemberment, and long term care insurance.

CareNavigator and FutureBright (“we” “us” “our”) want to inform individuals about why we need their personal information and how and why we handle their personal information.

We obtain consent to the collection, use and disclosure of personal information, except where inappropriate.

We state the purpose for which information is collected, used or disclosed at or before the time information is collected.

When we do collect personal information, we collect only that personal information which is necessary.

Typically information is collected, used and, if necessary, disclosed by us to process requests to obtain CareNavigator services or FutureBright's related insurance products.

From time to time we may request information from individuals that we indicate as being “optional”. This information is not required for insurance purposes but may allow us to serve individuals better and to offer our customers upgraded or additional products and services. An individual does not have to provide such optional information in order to obtain our products and services or any upgrades thereto.

Organisations may provide us with lists, or they may communicate the availability of our products and services to their customer or members. These organizations are responsible for obtaining appropriate consent to the use or disclosure of personal information for this purpose.

We do not sell our customer lists. However, information is shared between Care Navigator and FutureBright for marketing purposes. This sharing of personal information is optional.

An individual may decline or revoke consent to the optional collection, use and sharing of personal information described above, by contacting us at the address set out below.

We try to ensure that the information we collect and use is accurate, up-to-date, and as complete as possible. However, we rely on individuals to disclose all material information to us and to inform us of any changes.

We do not retain personal information unless we need it. If you apply for a Care Navigator service or a related FutureBright insurance product, your personal information will be forwarded on to the service provider or insurer, as the case may be. We will not keep copies of this information other than the information you provide us to process service fees or premiums.

Individuals may request to access and correct personal information in our possession. An individual may make this request by writing to our Privacy Officer at the address set out below. We may charge a reasonable fee in advance for copying and sending information requested and to which there is a right of access.
